(Free Analysis For Practice Owners Only) -- for use in obtaining a free preliminary analysis providing our thoughts, suggestions, sale price estimate range, “sellability” analysis, and preliminary structure / approach recommendations in regards to your project, such as: practice sale, associate addition, sale planning, Staged Equity Plan, and / or structuring the sale of your practice to an employed associate.

This formed is to be used by dentists that are thinking of adding an associate, and would like to have our thoughts as to whether their practices are busy enough to successfully add an associate, and  our thoughts as to what level of profitability they may realize from an associate addition.

a) This form is for use by dentists who wish to know more about practices that our clients are offering for sale.

b) It provides information about the inquiring dentists that assists us in determining whether any of the practices listed for sale will match with their career goals and location objectives.

c) It also provides information on the inquiring dentists’ experience so that we know more about the transition structure that might be necessary to increase their chances for a successful transition into ownership and operation of a practice.

d) And, it provides the inquiring party’s agreement to treat detailed information about a practice, that may be provided to him / her, with the appropriate level of confidentiality.

a) This form is for use by dentists who want to learn more about associateship positions that are available through our Company.

b) The form gathers information that assists us in advising the  inquiring dentists as to whether the positions available are appropriate given their career goals and experience.

This form guides you in gathering the information required to:

i)        Obtain a Practice Appraisal from our Company; or,

ii)      List your practice for sale with our Company.

This form is used to obtain practice appraisals.  Please do not submit this Agreement without first contacting our Company to assure that your practice location and appraisal needs fit within the guidelines of our Company’s appraisal assignment acceptance policy.  (Ask to speak with George D. Stollings, D.D.S.)

1. Those submitting this Agreement will also need the Information Collection Questionnaire available elsewhere on this page of our web-site.

2. This version is for appraisals to be used in sale planning, or in establishing a practice’s value and sale price for an impending sale, buy-in, or other form of ownership transaction;

3. It is not to be used by dentists who are simultaneously listing their practices for sale with our Company.  (Dentists listing practices for sale with our Company are eligible for an appraisal fee discount.  For details, see the Non-Exclusive Practice Sale Listing Agreement file found elsewhere on this page.);

4. This version is not for use in obtaining appraisals for divorce property settlements or other legal actions.  In such cases, contact our Company.

This agreement is for use by those wishing to list their dental practices for sale with our Company.  Please do not submit this Agreement without first discussing it with our Company, to assure that your geographic location, practice size, and sale time-frame fits within the guidelines of our Company’s listing acceptance policy.  (Ask to speak with George D.
